Community Support Worker

Salary: £25,760 to £27,476 a year (per annum)

Contract: Permanent, Full-time

Job Summary

Under the direction and supervision of senior practitioners to assist in the provision of care and treatment of clients in a community setting. Working with individuals experiencing mental health difficulties. To provide care, intervention and telephone and face to face support to individuals who are experiencing an acute mental health/psychological crisis. To assist care co-ordinators or key workers to assess, plan, implement and evaluate individual care plans.

Main Duties of the Job

  • Clinical: Maintain accurate electronic records in line with Trust policy.
  • Feed back changes in client's mental state or social circumstances, analysing risks and taking appropriate action.
  • Provide telephone/face to face support under supervision.
  • Deliver medication to clients.
  • Use petty cash for client activities in line with Trust policies.
  • Link with employment and benefit agencies.
  • Plan and organise own daily work activities.
  • Provide advocacy or links with advocacy organisations.
  • Promote opportunities for service users to develop friendships.
  • Contribute to support for carers and signposting.
  • Professional: Participate in regular supervision and annual appraisal.
  • Attend mandatory training.
  • Address training requirements with supervisor.

Job Responsibilities

  • Actively support service users in medication management within concordance framework.
  • Ensure physical health needs are met including advice and health education.
  • Engage with carers and complete Carers assessments.
  • Support senior staff in health assessments and delivery of care.
  • Assist with personal care needs such as bathing and washing.
  • Participate in delivery of care as part of Mental Health and Social Care Health Service.
  • Follow Code of Conduct for Health Care Assistants/Support Workers.
  • Receive supervision and partake in reflective practice/training.
  • Opportunity to undertake Level 3 Healthcare Support Worker Apprenticeship alongside substantive post.

Person Specification

Essential

  • NVQ Level 3 or equivalent competency based training in relevant health or social care area.
  • Knowledge of mental health.
  • Own car and full driving licence.
  • Communication skills.

Desirable

  • Experience of working in the community.
  • Experience of lone working.
