Band 6- Nurse/Paramedic Islington Urgent Community Response

Job Summary

The post holder will work in the Islington Urgent Community Response Team (IUCRT) as part of the multi-disciplinary team working across the Rapid Response and Hospital at Home pathways, supporting hospital discharges and admission avoidance. Key partners are [Employer hidden — sign up to reveal] and North Middlesex University Hospital Emergency Departments, assessment units, acute wards, community teams including district nurses and long term condition teams, London Ambulance Service and GP's within the Boroughs of Islington and Haringey.

Main Duties of the Job

  • Provide specialist, high quality, expert nursing/paramedic care for patients suitable for Urgent Community Response care.
  • Initiate agreed range of diagnostic tests.
  • Refer to other disciplines/professions when required.
  • Support patients to self-manage and make informed choices.
  • Act as named nurse for designated patients.
  • Ensure nursing documentation is maintained to required standards.
  • Promote evidence-based practice.
  • Participate in clinical supervision.
  • Ensure high standards of care.
  • Advise patients on health promotion and illness prevention.
  • Be responsible for intravenous access and administration of prescribed medication for patients requiring short-term antibiotics.
  • Collaborate with medical colleagues to ensure appropriate investigations and procedures.

Job Responsibilities

  • Promote the service and support clinical staff to facilitate safe, effective transfer of patients.
  • Work with and be responsible for case management of patients identified for Rapid Response and Hospital at Home.
  • Attend multidisciplinary meetings and cascade information.
  • Record data for performance monitoring.
  • Understand IUCRT protocols and pathways.
  • Advise on discharge options to patient, family and MDT.
  • Provide follow-up process for case managed patients.
  • Manage and coordinate patient care.
  • Develop patient condition specific management plans.
  • Attend internal and external meetings as representative of the Trust.
  • Actively participate in development of the IUCR service.
  • Assist in development of IUCRT pathways and protocols.
  • Gain real time feedback from service users.
  • Contribute to summary report and clinical audit.
  • Act within NMC/HCPC code.
  • Undertake further educational qualifications.

Person Specification

Education / Qualifications

  • Essential: RGN or Paramedic with current NMC/HCPC Registration
  • Desirable: Post registration education in a relevant area

Knowledge & Experience

  • Essential: Portfolio of varied Clinical experience, Experience in Community/ A and E/ Care of the Elderly/ICU
  • Desirable: Advanced Clinical Assessment Module or willing to work towards, Knowledge of current issues in the NHS

Skills & Abilities

  • Essential: Well-developed communication skills, Community Nursing skills or willingness to expand, Administration of IV medicines
  • Desirable: Diagnostic skills
