Job Summary
[Employer hidden — sign up to reveal] is seeking an experienced Advanced Clinical Practitioner to join our clinical team and undertake autonomous clinics as part of the GP rota. [Employer hidden — sign up to reveal] is a large, multi-site GP partnership serving approximately 49,000 patients across six sites in North Swindon. Our multidisciplinary team includes GPs, nurses, pharmacists, paramedics, physiotherapists and care navigators working together to manage both same-day demand and ongoing care for our patients.
Main Duties of the Job
- Undertake autonomous clinical assessment and management of patients as part of the GP rota
- Assess and manage patients presenting with undifferentiated and undiagnosed conditions
- Undertake clinical examinations, formulate differential diagnoses and implement management plans
- Prescribe where appropriate, arrange and interpret investigations, make referrals
- Contribute to same-day demand and routine GP workload
- Accurate clinical documentation, coding and administrative tasks
- Work autonomously with support from GP team and allocated GP mentor

Person Specification - Essential
- Registered healthcare professional with NMC, HCPC or equivalent
- MSc in Advanced Clinical Practice (or equivalent)
- Independent prescribing qualification
- Significant post-qualification clinical experience
- Experience assessing/managing undifferentiated conditions
- Autonomous clinical consultation and decision-making
- Strong communication and interpersonal skills
