Job Summary
This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.
Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening. Furthermore, they embed population health management approaches within general practice, targeting health inequalities and improving access.
The post-holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Job Responsibilities
- Infection Prevention and Control (IPC) - Support the IPC Lead and work in accordance with the practice IPC Policy.
- Patient Assessment and Consultation - Undertake holistic assessments of patient needs through consultation.
- Long-Term Condition (LTC) Management - Contribute to management of long-term conditions, participating in quality improvement.
- Care Management for Frail and Housebound Patients - Develop and embed effective care management programmes.
- Public Health, Screening, and Health Promotion - Support immunisations, vaccinations, and cervical screening.
- Population Health Management - Embed population health approaches to reduce inequalities and improve access.
- Evidence-Based Clinical Care - Deliver routine nursing care in line with NICE guidance and local protocols.
- Education and Support - Mentor new-to-practice nurses and empower patients.
- Clinical Audit, Research, and Governance - Participate in audits and update protocols.
- Working to Protocols and Scope of Practice - Work within national/local protocols and refer appropriately.
- Enhanced and Additional Services - Support enhanced services, lead public health campaigns, provide travel health advice.
- Safeguarding and Legal Compliance - Adhere to legislation relating to Safeguarding Adults and Children.
- Incident Management and Complaints - Participate in complaint management and learning from incidents.
- Mandatory Training and Appraisal - Complete mandatory training and participate in annual appraisal.
- Clinical Record Keeping and Coding - Maintain accurate records using SNOMED CT.
- Practice Targets and Registers - Contribute to QOF targets and maintain disease registers.
- Clinical Skills and Direct Patient Care - Provide wound care, Doppler assessments, chaperone, manage emergencies.
- Teamwork and External Liaison - Liaise with external services for patient support, maintain safe environment.
