CLINICAL PHARMACIST

Main duties of the job

You will work in a supportive, friendly environment with our integrated team of Doctors, ANP's, Clinical Pharmacist, Paramedic, Social Prescriber, Care Co-ordinator, Social Prescriber, Mental Health Nurse, Specialist Diabetes Nurse and Nursing team. The whole team works together to provide an innovative and caring service where patients are truly at the heart of everything we do.

Job responsibilities

  • Supporting patient care: Collaborating with clinical teams to provide best practices and mutual support.
  • Medication reviews: Conducting clinical medication reviews to manage patients with complex polypharmacy and chronic conditions.
  • Patient education: Providing clinical expertise and educational resources on medication sand treatment pathways.
  • Administrative support: Managing prescription queries, conducting audits, and ensuring safe prescribing practices.
  • Clinical leadership: Overseeing medicines optimization and quality improvement initiatives within the practice.
  • This role is pivotal for enhancing patient outcomes and operational efficiencies in general practice settings.

Person Specification

Experience

  • Desirable: Experience of working in a GP practice preferred.

Qualifications

  • Essential: Masters in Pharmacy. Need to have completed Primary Care Pharmacy Education Pathway.
  • Desirable: Prescriber preferred, but not essential.
