Job Summary

This post is based in the home dialysis department within nephrology and transplant. Your role will include supporting nephrology patients to undertake their own treatment of peritoneal dialysis or haemodialysis in their homes. This is aligned to Welsh Renal Network strategic priority of increasing access and uptake of home dialysis.

The Home Dialysis Team supports patients who are able to undertake their own haemodialysis or peritoneal dialysis in their home. It is a developing service looking to expand to support more people with the option to dialyse independently within the community. We are looking for a patient focused, enthusiastic staff nurse to join the multi-disciplinary team. Ideally, we would like someone with dialysis knowledge and skills and/or community experience but training will be given with an induction tailored to the requirements of the successful candidate.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main Duties of the Job

The successful candidate would then work within the David Thomas Dialysis Unit (DTU), University Hospital of Wales (UHW), the home training unit, Cardiff south dialysis unit and individual patient homes as required to meet the needs of our patients.

The small but expanding team sits within the Nephrology and Transplant Directorate where you have an opportunity to see the whole patient journey through various renal replacement therapies including the only Transplant Centre in Wales. You would also work closely with the Chronic Kidney Disease and Vascular Access Clinical Nurse Specialists. This is an exciting opportunity to make a difference within the multi-disciplinary team developing a service that allows patients to live more independently with a chronic illness.

It will be required that the successful candidate has access to transport and is able to travel as required to the South East Wales dialysis units and patient homes. To be able to assist and support the Team Leader with service improvement projects, help maximise potential of all the team, participate in staff education and development for the whole directorate.
