Home Care Practitioner - Keswick & Solway ICC
[Employer hidden — sign up to reveal] is seeking a Band 3 Home Care Practitioner to work within the Keswick & Solway Integrated Care Community (ICC). This part-time role (30 hours per week) focuses on providing rehabilitation-focused care in patients' own homes to support early hospital discharge and prevent unnecessary admissions. The post holder will be an active member of the multidisciplinary team, undertaking personal care, clinical observations, and delegated clinical duties. The ability to travel independently across a wide rural area and work a rota covering 8am-9pm over 7 days is essential. Closing date: 19 May 2026.
Main Duties of the Job
As a Home Care Practitioner within the ICC, you will:
- Undertake personal care activities and delegated clinical care duties including clinical observations and specified clinical skills as directed by registered professionals.
- Work with patients in their own homes to provide short-term support, aiming to maximise independence, increase confidence, and improve quality of life.
- Support Palliative care patients ensuring high quality, dignified care.
- Be an active member of the multi-disciplinary team (MDT) with close working relationships with local health and social care teams.

Person Specification
Qualifications
- Essential: Numeracy and Literacy Level 2; National Care Certificate or commitment to complete within 12 weeks; knowledge of data protection and health and safety legislation; Level 3 vocational qualification or equivalent experience in clinical care duties.
- Desirable: Additional training for Clinical Support Workers.
Skills & Aptitudes
- Essential: Excellent interpersonal, communication and active listening skills; time-keeping, planning and prioritisation; basic IT skills; analytical and judgement skills; attention to detail; ability to use initiative; compassionate values; ability to work independently and as part of a team; personal resilience.
- Desirable: Keyboard skills; venepuncture.
Knowledge and Experience
- Essential: Experience of working in health or social care; experience of undertaking and recording clinical observations; working knowledge of care legislation; understanding of accurate documentation; insight into carer behaviours; knowledge of Safeguarding; understanding of GDPR confidentiality.
- Desirable: Familiarity with NHS and social care policies; computer skills; nursing policies and procedures; experience of working in the Trust previously.
