Job Summary
To deliver a safe, effective, and patient-centred pharmacy and medicines service that supports the health needs of young people in a secure environment, in line with national standards, NHS guidelines, and relevant professional and local policies.
Main Duties of the Job
- Undertake medicines reconciliation for new admissions and transfers, resolving discrepancies.
- Support medicines reviews and patient counselling.
- Support patients with substance misuse and withdrawal concerns.
- Conduct audits and KPI reporting, monitor formulary compliance.
- Ensure safe handling and accurate recording of controlled drugs.
- Work collaboratively with multidisciplinary teams and external agencies.
- Contribute to clinical governance and continuous service improvement.
- Function autonomously within scope of competence, maintain GPhC requirements.

Job Responsibilities
- Medicines Reconciliation & Admission Screening: comprehensive reconciliation for new receptions and transfers, identify and resolve discrepancies.
- Medicines Optimisation & Patient Support: medication checks, support MURs and patient counselling, advice on in-possession medicines.
- Substance Misuse & Withdrawal Management: recognise and escalate signs, support patients with substance misuse concerns.
- Audit, KPI Reporting & Formulary Oversight: conduct audits, monitor formulary, provide data for KPI reports.
- Drug Safety & Controlled Drugs: promote medicines safety, ensure compliance with controlled drug legislation, maintain records.
- Collaborative Working & Governance: communicate with MDT, participate in clinical meetings.
- Professional Practice & Development: function autonomously, maintain CPD, meet GPhC requirements.
Person Specification
Qualifications
Essential: Registered Pharmacy Technician with GPhC, NVQ Level 3 / BTEC in Pharmacy Services (or equivalent), accreditation in medicines reconciliation or accuracy checking (desirable). Excellent organisational skills, ability to work independently and as part of a multidisciplinary team, strong communication skills, commitment to medicines safety.
Desirable: Previous experience within a secure setting, SystmOne experience.
