Salary: £49,387 - £56,515 per annum (Band 7) | Closing date: 24 May 2026
Job Summary
This role requires the clinician to work as an independent, autonomous practitioner as part of Managing Memory Together to support service users with a dementia diagnosis in primary care - ensuring appropriate levels of support, advice and therapeutic interventions are delivered in a coordinated way.
Main Duties
- Support clinical decision making on diagnosis, treatment and follow up care for patients diagnosed with dementia.
- Co-chair ICS dementia co-diagnosis MDT meetings across allocated PCNs.
- Act as lead professional for people with dementia allocated to own caseload.
- Support other lead professionals across services.
- Provide highly specialist clinical advice to primary care teams.

Person Specification
Essential
- Professional/clinical knowledge acquired through degree, supplemented by specialist training. NMC Registration (First Level Nurse).
- Evidence of study at master's level or willing to undertake further study at master's Level.
- Significant Post Registration Experience of working with service users diagnosed with a dementia.
- Experience of primary care work and/or working in the Community & of managing a designated caseload.
- Evidence of clinical assessment skills in current clinical practice and demonstrates highly developed listening skills.
- Applies and demonstrates contemporary development in mental health care particularly dementia.
Desirable
- Other post registration courses e.g. Assessment and Management of a potentially aggressive person.
