Community Dementia Practitioner – Managing Memory Together

Job Summary

This role requires the clinician to work as an independent, autonomous practitioner as part of Managing Memory Together to support service users with a dementia diagnosis in primary care - ensuring appropriate levels of support, advice and therapeutic interventions are delivered in a coordinated way.

Main Duties

  • Support clinical decision making on diagnosis, treatment and follow up care for patients diagnosed with dementia.
  • Co-chair ICS dementia co-diagnosis MDT meetings across allocated PCNs.
  • Act as lead professional for people with dementia allocated to own caseload.
  • Support other lead professionals across services.
  • Provide highly specialist clinical advice to primary care teams.

Person Specification

Essential

  • Professional/clinical knowledge acquired through degree, supplemented by specialist training. NMC Registration (First Level Nurse).
  • Evidence of study at master's level or willing to undertake further study at master's Level.
  • Significant Post Registration Experience of working with service users diagnosed with a dementia.
  • Experience of primary care work and/or working in the Community & of managing a designated caseload.
  • Evidence of clinical assessment skills in current clinical practice and demonstrates highly developed listening skills.
  • Applies and demonstrates contemporary development in mental health care particularly dementia.

Desirable

  • Other post registration courses e.g. Assessment and Management of a potentially aggressive person.
