Community Support Worker

[Employer hidden — view at passion-project.co.uk] is hiring a Community Support Worker for the Discharge to Assess team. The role involves supporting older adults with mental health and physical health needs in the community. Closing date: 08 May 2026.

Following a successful pilot project and the progression of the previous post-holder, we are looking to recruit a Community Support Worker for Discharge to assess team. The team covers both North, East, south and West Herts. You will be working alongside a band 6 either allocated to the East and North of the County or the West and South of the County

South CAMHS is a multi-disciplinary team providing mental health assessment and intervention to young people aged 0-18 presenting with complex mental health difficulties.

Our team consists of psychiatry, clinical psychology, systemic therapists, child psychotherapist, clinical social workers, nursing, CAMHS practitioners and support workers.

Main Duties of the Job

As part of this role you will be working closely the Community Psychiatric Nurse allocated to that area, to support a smooth transition from hospital to care home.

This role is both clinic based as well as involving significant outreach and community based support.

We are looking for an enthusiastic and dynamic individual who can work flexibly to support our service user that are frail and require a period of assessment in one of our D2A beds.

It is essential for the candidate to have experience of working with individuals with mental health difficulties and complex physical health

As part of this role, you will receive regular supervision and support and will have opportunities to further develop your skills, especially in physical health.

Job Responsibilities

The post holder will work under the supervision and direction of a Registered Nurse, but will manage their own diary and caseload. They will regularly report back their findings and assessment to the Registered Nurse.

The post holder will contribute to assessment of both physical and mental health needs. The post holder will understand the relationship between physical and mental wellbeing.

To work with other agencies to deliver a systems approach in ensuring service users needs are assessed and optimised during the Discharge to Assess period.

The post holder will support the assessment of service users who have been identified as having a mental health need. They will take and record mental state examinations, as well as physical health checks including blood pressure, pulse, weight and BMI. They will also carry out venepuncture for required blood tests.

Person Specification

Knowledge, Training, and Experience

Essential

  • Good standard of education (to include Maths and English GCSE grade C or above or equivalent)
  • Experience of working with Older people in either an acute or community setting
  • Experience of working with Older People with mental health conditions and complex Physical Health
  • Experience of working autonomously and using own initiative
  • Ability to manage distressing and challenging situations

Desirable

  • Trainee Nursing Associate qualification
  • NVQ in Health and Social Care

  • Excellent communication skills, verbally and in writing
  • Ability to feedback and discuss cases within the multi-disciplinary team
