Lower Limb Co-ordinator
An exciting opportunity has arisen for an experienced Band 6 Community Nurse to take on the role of Lower Limb Coordinator within our Community Nursing Service. The post holder will lead on the assessment, management, and treatment of patients with lower limb conditions, particularly leg ulcers, delivering high-quality, evidence-based care. You will hold a defined caseload, working both autonomously and collaboratively within the wider multidisciplinary team.

Job Summary
The post holder will be an experienced band 6 community nurse, registered with NMC, responsible for patients allocated to the lower limb co-ordinator for leg ulcer management and treatment. The post holder will assess, manage, plan and deliver care, including being professionally and legally accountable. They will hold a caseload, support peers and team leader, and work both autonomously and as part of the integrated primary/community health care team, liaising with GPs, other professionals and agencies. The role includes clinic-based work and community visits (one day/week), educating new starters, and providing leadership to the community nursing team.
Main Duties
- Holistic assessment, planning, implementation and evaluation of care for patients with leg ulcers
- Monitoring and reviewing patient treatment plans
- Nurse Prescribing, Non-Medical Prescribing
- First contact assessment, care and referral
- Continuing care, rehabilitation and delivering National Service Frameworks
- Providing public health and health promotional programmes
- Compression therapy
- Caseload management
- Maintaining contemporaneous written and computerised records
- Leg Ulcer audits and Decontamination audits
