Job responsibilities

• Implement health promotion and wellbeing programmes
• Develop and implement individual care plans for chronic disease patients
• Manage and support patients at risk of long-term conditions
• Provide wound care
• Run specialist clinics (ear syringing, contraception, etc.)
• Collect specimens for pathology
• Provide travel advice and vaccinations
• Request pathology as necessary
• Process and interpret test results (e.g. cervical cytology)
• Provide LTC clinics and refer to secondary care
• Maintain accurate clinical records
• Implement call/recall system
• Implement immunisation programme
• Chaperone patients
• Support clinical team in emergencies
• Contribute to practice targets (QOF, local targets)
• Liaise with external services for safeguarding

Person Specification

Experience

Essential: Experience in general practice as a practice nurse, working autonomously, understanding of infection prevention and control.

Desirable: Range of primary care nursing duties, prescribing, medication reviews, supervisory/mentoring role.

Qualifications

Essential: Degree in nursing, registered nurse on NMC register.

Desirable: Qualified independent nurse prescriber, postgraduate diplomas in chronic disease management, cervical screening, spirometry.

Other Requirements

DBS check, occupational health clearance, NMC revalidation, evidence of CPD.
