FULL DESCRIPTION

[Employer hidden — sign up to reveal] Ltd is seeking a Rehabilitation Support Worker Level 2. This role involves providing contact, care, and support to clients in their homes and community. The position requires flexibility for shifts including evenings, weekends, and sleep-ins. Experience with brain injury or impairments is desirable.

Duties and Responsibilities

Administrative and Rehabilitation Support Provision as outlined in the job description.

  • Work flexibly according to client and company needs.
  • Liaise with Area Coordinator regarding rota and activities.
  • Maintain effective communication with client, family, and therapists.
  • Follow support plans and risk assessments.
  • Keep accurate daily records, complete money recording and medication charts.
  • Communicate concerns promptly and document accordingly.
  • Complete timesheets weekly, attend training and meetings.
  • Assist with rehabilitation: therapy programs, social skills, leisure, personal care, health monitoring, challenging behaviour management.
  • Ensure health and safety of client, maintain privacy and confidentiality.

Person Specification

Skills and experience required.

  • Experience supporting clients with brain injury or other impairments.
  • Experience with community-based rehabilitation programmes.
  • NVQ/Diploma level two/three in health & social care or equivalent.
  • Computer skills, good interpersonal and communication skills.
  • Ability to work alone and as part of a team.
  • Knowledge of health & safety, confidentiality.
  • Physical ability for moving and handling, good general health.
  • Car driver, available for flexible shifts including evenings, weekends, Bank Holidays, sleep-in duties.
  • Practical ability in social and domestic tasks, willingness to accompany client on short breaks/holidays.
