Rehabilitation Support Worker Level 2

[Employer hidden — sign up to reveal] Ltd - Main Purpose: To provide contact, care and support according to the needs and wishes of the client, to enable the client to function in their home and in the community to their maximum potential.

Duties and Responsibilities

  • Administrative: Work flexibly, liaise with Area Coordinator, maintain communication with client/family/therapists, follow support plans and risk assessments, keep accurate records, complete money recording and medication charts, communicate concerns, complete timesheets, attend training and meetings.
  • Rehabilitation Support Provision (Moderate dependency): Implement support plans, assist with rehabilitation to develop daily living skills including therapy programs, social skills, leisure & hobbies, daily living activities (personal care, household duties, meal prep, health monitoring, medication), maintaining safety.

Person Specification

  • Essential: Experience in rehabilitation care plans, NVQ/Diploma level 2/3 in health & social care, interpersonal skills, communication, teamwork, ability to work alone, confidentiality, physical ability for moving and handling, car driver, flexible shifts including evenings/weekends/Bank Holidays/sleep-ins, practical social/domestic skills, ability to accompany client on holidays.
  • Desirable: Knowledge of Cerebral Palsy/Acquired Brain Injury, community-based rehabilitation experience, current medication administration certificate, moving and handling certificate, food hygiene, first aid, health & safety knowledge, local area knowledge, dementia knowledge, experience driving wheelchair adapted vehicle.
