Practice Facilitator - Registered General Nurse

Job summary
We are seeking an experienced Practice Facilitator to help steer, drive, support and deliver service improvement within the Mental Health, Learning Disability and Neurodiversity (MHLDN) & Wellbeing Services directorate in [Employer hidden — sign up to reveal].

Main duties of the job
- Deliver training sessions, ensure staff competency, and offer assistance when other team members are unavailable.
- Work within the scope of professional practice and [Employer hidden — sign up to reveal] policies.
- Maintain key working relationships with managers, service heads and leads.
- Establish and uphold proactive connections with development and training providers including [Employer hidden — sign up to reveal] Network, NHS Trusts, Plymouth City Council, and private/voluntary sector stakeholders.
- Work within scope of practice across the organisation should the need arise.

Job responsibilities
- Plan, implement, deliver and evaluate clinical and essential training using national and local guidelines.
- Support delivery of training programmes led by other clinical educators.
- Facilitate practice supervision and peer learning groups across Community Nursing teams.
- Provide expert clinical leadership and support to all staff groups.
- Act as a role model for Student Nursing Associates and nurses.
- Maintain training equipment and stock levels.
- Ensure quality service delivery by developing and reviewing workforce competencies.
- Work autonomously in the community setting, managing own workload.
- Perform comprehensive holistic assessments and physical examinations.
- Initiate and support medicines management incidents.
- Develop and facilitate peer support groups.
- Lead on practice supervision development and implementation.

- Ability to assess acute/chronic patient conditions

- Understanding of Clinical Governance
- Evidence of appropriate clinical competence and ongoing professional development
Essential Experience & Qualifications
- Registered Nurse educated to degree level
- Community experience
- Teaching skills
- Mentorship Module or equivalent/willingness to undertake
- Clinical and Management skills
- Leadership skills

- Evidence of recent study
- Good multi-agency liaison skills
- Good time management and delegation skills
- IT literate
- Ability to work with difficult and complex situations
