Role Overview

The Nurse Practitioner will work in three blended specialities: Care Co-ordination (CCC), Single Point of Access (SPA), and the Oncology helpline and Telephone triage. [Employer hidden — sign up to reveal]'s mission is to keep the patient at the heart of all that we do, providing a safe and high-quality service.

  • CCC: Provides telephone-based service to primary, urgent and emergency care colleagues across Shropshire and Powys, coordinating admissions and pathways.
  • Oncology: Dedicated helpline for patients under Oncology and Haematology services, following National Guidelines endorsed by MacMillan Cancer Support and UKONS.
  • Telephone triage: In and out-of-hours service triaging calls from 111 and other OOH providers, requiring autonomous clinical assessment and critical thinking.
