Advanced Clinical Practitioner - Urgent Care
[Employer hidden — view at passion-project.co.uk] is hiring an Advanced Clinical Practitioner for urgent care at Royal Preston Hospital and Chorley & South Ribble Hospital. Salary: £55,157 to £61,850 a year plus rate enhancements for evening & weekend working. Permanent, full-time or part-time.
Main Duties
- Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
- Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
- Provide information and advice on prescribed or over-the-counter medication and medication regimes, side-effects and interactions.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Refer and discharge safely ensuring that safety-netting advice is offered.
- Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

- Experience of autonomous practice
