FULL DESCRIPTION

Reablement Worker

[Employer hidden — sign up to reveal] is looking for compassionate, motivated Reablement Workers to join our Discharge to Assess (D2A) team supporting people across South Somerset. This role plays a vital part in helping people return home safely from hospital, regain independence, and rebuild confidence following illness or a hospital stay.

Location: South Somerset (Yeovil, Wincanton, Somerton, Castle Cary, Templecombe, Chard, Crewkerne, Ilminster, South Petherton, Martock)

Salary: £14.07 – £15.12 per hour (£29,588.52 for 39hr week)

Hours: 39 hours per week, rolling 2-week shift pattern (Early, Late, Split) with every other weekend

About the role

As a Reablement Worker within Intermediate Care Pathway 1, you will:

  • Support people recently discharged from hospital to settle safely back into their own homes
  • Encourage independence through short‑term, goal‑focused support
  • Assist with personal care, daily living tasks, mobility, and wellbeing
  • Work closely with therapists, health colleagues, and families as part of a multi‑disciplinary team
  • Review progress regularly and adapt support as independence improves

About you

We're looking for people who are:

  • Caring, patient, reliable, enabling and conscientious in record keeping
  • Passionate about supporting independence and recovery
  • Able to work flexibly, including mornings, evenings and every other weekend
  • Competent and confident working independently and as part of a wider team
  • Good communicators with a positive, can‑do attitude

Experience in care is essential (Minimum of 1 Year). A full UK driving licence and access to a vehicle is required due to community‑based working.
