Discharge Facilitator
The Community Discharge Assessment Team (CDAT) at [Employer hidden — sign up to reveal] aims to prevent avoidable hospital admissions and works closely with the multi-disciplinary team. This role requires registration in Nursing (NMC), Occupational Therapy (HCPC), or Physiotherapy (HCPC). Discharge Facilitators assess patients, coordinate discharges, and liaise with community services. Hours available: 37.5 and 22.5 per week.
About the Role
The service operates 7 days a year 08:00–20:00 (Mon–Fri) and 08:00–18:00 (Sat–Sun & Bank Holidays). Discharge Facilitators will be predominantly based in the Emergency Department (ED), Same Day Emergency Care (SDEC), or acute assessment wards. They attend MDTs, download patient lists from PPM+ and Symphony Live, and produce referrals to Neighbourhood Nursing Teams, Adult Social Care, Homeless & Housing Teams, Community Rehabilitation Beds, and Third Sector Organisations.
