Hours: 37 hours per week (full time) remote and part on-site working, flexible hours

The role and service / project: We are looking for a motivated, passionate and experienced WellFamily Worker with an impressive record of accomplishment of delivering high quality 1 to 1 services to meet the needs of service users from a Primary care setting. You will strive for excellent standards, robustly ensuring a quality, outcomes driven and accessible service. By joining [Employer hidden — sign up to reveal]’s WellFamily Plus service, you will ensure good quality holistic assessment is for people 16 and over, using emotional and practical interventions to meet the health and wellbeing outcomes for adults, children, young people and families. This is a short-term intervention model – offering up to eight sessions. It is a well-established model in City and Hackney and embedded within the 42 GP practices across the borough.
Your impact: Collaborating with health professionals and promoting co-production with service users, you will provide 1:1 personalised support to individuals and identify wider issues that impact health and wellbeing. You will address individuals’ unique needs by designing bespoke wellbeing plans and establishing partnerships, helping individuals improve their health and wellbeing, making positive, long-lasting lifestyle changes and play a pivotal role in diversifying and personalising care in local communities.
Your skills: You will have a strong track record in service delivery and a focus on quality, performance, safeguarding and record keeping. With a client-facing approach you will be passionate about engaging our service users in all aspects of our work including co-production. We want to hire a ‘can-do’ and energetic Wellbeing Plus Practitioner at a senior level who will build and sustain positive working relationships with key stakeholders while developing partnerships that will continue to evolve and effectively meet the needs of everyone the service supports.
