Key responsibilities
- Provide person-centred care and practical support (daily living, routines, community access)
- Support with personal care where required, respecting privacy and dignity
- Accurate daily notes and incident reporting
- Follow risk assessments, safeguarding procedures and support plans
- Work collaboratively with colleagues, families and professionals
