Young Persons Community Support Navigator

Location: Northside Exchange, Kirkstall, Leeds
Salary: £29,050 - £31,619 per year (full-time, fixed term)

Job Description
The SHAP programme will support young people aged 18-25 to bridge the gaps in provision for young people who typically ‘fall through the net’ due to the complexity of their needs. Through SHAP, [Employer hidden — sign up to reveal] Navigators will strive to have enhanced engagement with young people, taking a no wrong door approach and continuing support despite periods of crisis and disengagement. The SHAP offer will strive to improve the mental health of young people in Leeds, support them to address their substance use and prevent rough sleeping.
