Community Support Worker
[Employer hidden — sign up to reveal] is seeking a motivated Community Support Worker to join the Primary Care Home Hub based at Beccles Hospital. This is a part-time, permanent role at Band 2, with a salary of £25,272 per year. The role requires working 34.5 hours per week (3 x 11.5 hour shifts) including weekends and bank holidays.
Job Summary
The team delivers Community healthcare to patients in their own home in the Waveney area (including Beccles, Bungay, Halesworth, Southwold and Kessingland). This role focuses on reablement interventions, delivering care and support to promote independence, prevent unnecessary hospital admission, and support end of life care. The Primary Care Home is a multidisciplinary team of Nurses, Therapists, Assistant Practitioners, Health Care Assistants, and Community Support Workers.
Main Duties, Tasks & Skills Required
- Work with patients towards self-care and independence.
- Support and deliver aspects of activities of daily living, including dietary and fluid intake, continence management, toileting assistance.
- Take, record and report blood pressure, respiration, pulse rate, oxygen saturation levels, urinalysis.
- Undertake skin assessments and like-for-like dressing changes.
- Provide walking aids and small aids/equipment.
- Prompt, assist and manage medication as per MAR chart.
- Participate in end of life care.
- Ensure high standard of record keeping in line with [Employer hidden — sign up to reveal] standards using SystemOne.

Person Specification
Qualifications
Essential: Evidence of personal development, Care Certificate (or willing to complete). Desirable: Training in communication skills, Level II NVQ or equivalent in Health/Social Care.
Experience
Essential: Previous experience in NHS/Social Care/Voluntary organisation, awareness of needs of older people. Desirable: Community setting experience, multidisciplinary team work, understanding of reablement.
Additional Criteria
Essential: Good interpersonal, verbal, listening, time management, IT, and written skills. Willingness to travel across community, work without direct supervision, confidentiality awareness, approachable, reliable, embrace values. Desirable: Willingness to learn new skills.
