Job Introduction

Department of Health & Wellbeing, Adult and Community Services, Operational Services. Band SO1: £33,366-£35,235 SPC 23-25. x2 37 Hours. Base: Bradford Hospitals and Community.

We are looking for people who can:

  • To look at the Home First vision and or signpost to Voluntary services and the use of assistive technology, preventing the need or reduced need for ongoing care and service provision.
  • Work in the community as part of the Home First Assessment Support Team (HFAST) carrying out the true discharge to access model.
  • To support and undertake the Home First Assessment on people (many of whom will have complex conditions including dementia) on behalf of the local authority beds and Enablement services and independent Nursing Homes, thereby facilitating a safe and timely discharge from hospital.
  • To make clear and transparent recommendations and proposals as to the future destination of care in the best interests of the person following the completion of an assessment.
  • Benchmark / Develop / review generic based paperwork to facilitate a holistic and efficient assessment of the person and their needs in partnership with service providers.
  • To negotiate the most appropriate ways of achieving the objectives identified which also encompasses the ability to identify competing demands and prioritise requirements.
  • The ability to liaise with other professionals and agencies involved in the provision of care this could be by attending the appropriate care forums to build enduring positive relationships with service providers to embed the trusted assessor role.
  • The ability and ownership to Access/ deal with / understand and navigate all systems for example: - inclusive of Power BI, System One, to enable reviewing, monitoring and evaluating the effectiveness of the service, systems and the documentation.
  • Work in partnership with partners, discharge coordinators and social workers to achieve optimisation of resources and ensure effective transition of people into the most appropriate resources.
  • Provide independent perspective from those of the service provider or hospital personnel to ensure provision is unbiased and effective.
  • To be aware of the different cultural needs of the people and to develop and deliver culturally sensitive processes – alongside develop a system for collating and storing records including outcomes and learning to ensure effectiveness and openness and transparency.
  • Ensure records are of a high-quality standard for individual’s outcomes and learning.
  • Develop and gather a range of key performance indicators in order to demonstrate the effectiveness of the trusted assessor role – this will include the production of reports as required.
  • Demonstrate a commitment to developing the service and role by researching and sharing good practice knowledge and skills - liaising with comparator authorities.
  • Attend and contribute to any relevant meetings related to improving delayed transfers of care.
  • Experience of working in a senior position and completing assessments.

Applicants must have

  • The NVQ level 3 Diploma in Adults Health & Social Care.
  • The post-holder must be able to work weekends, and some bank holidays as required by the needs of the Service.
  • Shift working between the hours of 7.00 – 22.00.
  • Full UK driver’s license and access to a car.
