Job Summary

Exciting Job Opportunity for Practice Nurse at [Employer hidden], Hereford (WBC PCN). We are expanding our nursing team and seeking a Practice Nurse to join our forward-thinking, happy community-based GP surgery. Main duties of the job

  • Deliver evidence-based nursing care to patients with acute and chronic conditions.
  • Deliver clinics for long term condition management.
  • Perform reviews in line with protocols and best practice.
  • Provide immunisations, vaccinations, cervical cytology, wound care, travel health advice, and lifestyle support.
  • Ensure accurate clinical documentation in line with NMC standards.
  • Work with the wider MDT to ensure coordinated care and appropriate referrals.
  • Participate in protocol development, audits, and service evaluation when required.
  • Engage in professional development, mandatory training, and uphold the NMC Code.

Job responsibilities

  • Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
  • Consult with patients and their families to undertake assessments of patient need
  • Support in the long-term condition management within the organisation
  • Develop, implement and embed an effective care management programme for the frail and housebound patients
  • Support public health and screening programmes
  • Embed population health management approaches
  • Provide routine nursing care in accordance with clinical based evidence
  • Support new-to-practice nurses and other healthcare professionals
  • Teach and advise patients and their families
  • Participate in clinical audits and research projects
  • Recognise boundaries of practice and refer appropriately
  • Support delivery of enhanced services

Person Specification

Knowledge

  • Understanding of policy developments related to primary care services
  • Understanding of evidence based practice
  • Knowledge of national standards (NSF, NICE)

Experience

  • Experience in managing chronic conditions in a nurse-led capacity
  • Understanding of Practice Nurse role in general practice
  • Experience implementing clinical protocols and guidelines
  • Experience with immunisations, wound care, cytology, and ABPI
  • Effective time management and ability to work independently
  • Confident communication of sensitive information
