Care Coordinator
Location: Wirral | Job Type: Full-time | Work Type: On-site

As a Care Coordinator, you will be a key member of our healthcare team, responsible for overseeing the planning and delivery of care services to patients or clients. Your role involves managing care schedules, coordinating with healthcare professionals, and ensuring the highest standard of care is provided. This position requires strong organizational and leadership skills, as well as a commitment to improving the quality of care.

Key Responsibilities:

  • Care Planning: Collaborate with healthcare professionals to develop and update individualized care plans for patients or clients.
  • Scheduling: Create and manage care schedules for clients, ensuring that all care visits are covered and delivered efficiently.
  • Team Coordination: Supervise and coordinate the activities of care staff, ensuring they are adequately trained and motivated.
  • Client Assessment: Conduct client assessments and reassessments to determine changing care needs.
  • Quality Assurance: Monitor and maintain the quality of care services, conducting regular audits.

Qualifications:
