About the Role

As a Care Coordinator, you’ll make sure our home care service runs smoothly every day. You’ll match the right Care Assistants to the right customers, ensuring every visit happens on time and to a high standard.

Your main responsibilities:

  • Create and manage daily and weekly rotas
  • Communicate clearly with carers, customers, and families
  • Handle last-minute changes calmly and efficiently
  • Maintain service quality and continuity for all clients
  • Support the Care Manager with compliance and reporting
  • Build strong relationships across the Wandsworth care team

What We’re Looking For
