District Nurse/Deputy Team Leader

An excellent opportunity is offered for an experienced Community Nurse to apply for the post of Band 6 District Nurse Caseload holder post . The successful applicant will hold responsibility for the care and management of a defined clinical caseload of patients within a very busy District Nursing Team . There are multiple demands in this exciting role due to the challenging nature of providing care for patients outside of the hospital environment.

An excellent opportunity is offered for an experienced Community Nurse to apply for the post of Band 6 District Nurse Caseload holder post . The successful applicant will hold responsibility for the care and management of a defined clinical caseload of patients within a very busy District Nursing Team . There are multiple demands in this exciting role due to the challenging nature of providing care for patients outside of the hospital environment.

Main duties of the job

The post holder will be responsible for providing support to the Team Leader in the day to day management of multidisciplinary/multi-agency packages of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. Assisting the Team Leader in the requirements of the nursing team by utilising effective management and leadership skills.

Responsible for the delegation of clinical nursing activities to appropriately skilled team members, ensuring that an efficient and effective service is delivered to the practice population.

Responsible for prioritising delegated home visits according to the patients' clinical need and geographical location.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
