We are seeking a compassionate Diabetes Specialist Nurse to join a GP clinic, providing expert, patient-centered care and clinical support.
Key responsibilities include:
- You will identify, manage and support patients that are risk of diabetes or have complex diabetes
- Develop care plans and prescribe medication
- Implement care plans and health care education to patients as well as healthcare practitioners
Requirements:
- Be able to prescribe (don’t need to titrate)
- Previous experience in Diabetes review
- Postgraduate diploma or equivalent
This opportunity offers:
- The hours: 9am till 6pm
- Shifts: 5 days a week
- Outside IR35
