[Employer hidden] has opportunities for Advanced Clinical/Nurse Practitioners to support a Home Visiting Service in York. In this role you will provide high-quality, patient-centred care to people in their own homes across York. This is a rewarding opportunity to work autonomously, support local GP practices, and deliver excellent clinical care to patients who need it most.

Role Overview:

• Conduct home visits for acute and chronic conditions
• Perform assessments, diagnoses, and management plans
• Prescribe safely and appropriately
• Work collaboratively with local GP practices and community services
• Deliver compassionate care to patients who may be frail, housebound, or unable to attend appointments

Required Qualifications & Experience

• NMC/HCPC Registration
• Master’s (MSc) in Advanced Clinical Practice or equivalent Level 7 qualification.
• V300 Independent Non-Medical Prescribing
• Experience in primary or urgent care
• Strong clinical decision-making skills
