[Employer hidden] have an exciting opportunity for Advanced Practitioners to support a GP Hub and OOH service in Hereford

We are seeking an experienced and motivated Advanced Practitioner to join our friendly and supportive Out of Hours/GP Hub service in Hereford. This is a predominantly face-to-face clinical role, providing high-quality urgent primary care to patients during across the week, primarily evening nights and weekends.

You will work as part of a multidisciplinary team delivering safe, effective and patient-centred care in a modern clinic environment.

Key Responsibilities:

  • Provide autonomous clinical assessment, diagnosis and treatment of patients presenting with urgent primary care needs
  • Manage acute and long-term conditions within your scope of practice
  • Prescribe safely and appropriately
  • Refer patients to other services as required
  • Work collaboratively with GPs, nurses, paramedics and operational teams
  • Maintain accurate and timely clinical records
  • Ensure patient safety and high standards of clinical governance

You will need:

  • A qualified Advanced Clinical Practitioner/Advanced Nurse Practitioner/Advanced Paramedic Practitioner
  • V300 Qualification
  • Registered with your relevant professional body (NMC/HCPC)
  • Experienced in primary care settings
  • Confident working autonomously in a face-to-face clinic environment
  • An excellent communicator with strong clinical decision-making skills
  • Committed to delivering compassionate, high-quality care
