Advanced Clinical Practitioner (ACP)

[Employer hidden] has an opportunity for Advanced Clinical Practitioners (ACP) to support a Telephone Triage service in Mansfield.

In this role you will be conducting clinical assessments over the phone to identify patient needs and provide appropriate advice or interventions. You will be signposting patients to the most suitable care pathways or services.

We are keen to speak with ACPs who hold a NMC/HCPC registration, have proven experience in telephone triage or similar clinical roles and have strong decision-making and communication skills. In this role, you may be required to conduct face-to-face assessments based on client requirements.

Requirements/Experience:

• Telephone Triage Experience
• NMC/ HCPC Registered
• MSc in Advanced Clinical Practice
• 6 months experience of acute care setting
• Proficient on Adastra and SystmOne
