Advanced Clinical Practitioner (ACP)

[Employer hidden] has opportunities for experienced Advanced Clinical Practitioner (ACP) to support Enhanced Access Clinics and Home Visiting services in Barnsley.

We are looking for ACPs who can work across both Enhanced Access and Home Visiting services to support a client in Barnsley.

The role is predominantly respiratory-focused, though you will also assess and manage a range of minor illnesses within a primary care setting. You will also carry out home visits to housebound patients as part of the home visiting team.

Shifts available: Monday to Friday, 09:00–14:00

Key Requirements

• MSc or PGDip in Advanced Clinical Practice (or equivalent)
• Strong respiratory experience (assessment, diagnosis, management)
• Independent Prescriber (V300)
• Extensive Primary Care Experience (GP OOH/HVS)
• Proven ability to work autonomously in primary care
• Confident in managing minor illness presentations
