Assistant Practitioner
The Southwark Older People & Physical Disabilities (OPPD) Service is seeking an Assistant Practitioner for their Community Review Team. Responsibilities include completing Care Act 2014 assessments, Carers assessments, and reviewing social care needs. The role involves support planning, working with complex issues, and safeguarding under supervision.
The Role
Older People & Physical Disabilities Service – Community Review Team
The Southwark Older People & Physical Disabilities (OPPD) Service provides a multidisciplinary approach to assessing and supporting individuals experiencing either a physical impairment, cognitive impairment or illness.
Within the OPPD service we have an Assistant Practitioner position available within our Community Review Team
Southwark Adult Social Care works in a strengths based manner focusing on people’s strengths and abilities to adopt an assets based approach to assessments. This enables us to support individuals to make greater use of their community resources and to maximise their independence.
As part of the team you will be responsible for completing Care Act 2014 assessments and determining whether or not an individual is eligible for services. You will also be asked to complete Carers assessments to ensure that we are supporting the numerous informal carers within the borough. In addition, you will be responsible for reviewing both the social care needs of these individuals and the social care services that they receive via Southwark Adult Social care.
To work with the person and other professionals as necessary in order to facilitate the stated functions of the Community Review Team. This includes the undertaking of proportionate assessments, ongoing support, support planning and proactively working with complex issues, family dynamics, legal issues, crisis, providing information and advice, transferring to another team if required and the provision of ongoing safeguarding, under direct supervision from a professional. Responsible to the Team Manager and Senior Practitioner.
