[Employer hidden — view at passion-project.co.uk]’s Leaving Care team is currently undergoing exciting change. Funded by the DfE Innovation programme, and working in partnership with Catch 22, we are trying to improve the outcomes of care leavers by exploring new ways of working. We are an enthusiastic and supportive team, who are passionate about working with young people. This is a fantastic opportunity to be part of, and help shape, the change in our service, and work to co-design the service with young people, local organisations and partners. We have recently recruited experienced practitioners in substance misuse, housing, vulnerable young people and youth work who are keen to join us on our journey and contribute to improving our service.

This role is new and focused on improving outcomes for care leavers by coordinating volunteering opportunities and sourcing additional support and resources from the wider community across Southwark. Currently, the service is setting up a mentoring scheme internal to [Employer hidden] to support care leavers into education, employment and training, which the successful candidate would lead and build on. The role is based in the recently established unaccompanied asylum seeker and 21+ team.

The successful candidate will need to be comfortable with reviewing data and holding focus groups with care leavers and colleagues to understand their needs, working collaboratively with other colleagues. You will then create links with local organisations (private, voluntary or other) to find out how these needs can be met. You will need to be able to demonstrate how new services can create impact and be sustainable, and monitor outcomes over the long-term. The successful candidate will also hold a caseload of 10 care leavers and act as a personal advisor to them.
