Community Connector

We are looking for committed and enthusiastic Community Connectors to help make a difference to vulnerable adults in the community. You will be joining a friendly, professional, and supportive team; your work with Council colleagues and other key partners will help people to achieve positive outcomes. You will be joining a friendly, professional, and supportive team; your work with Council colleagues and other key partners will help people to achieve positive outcomes. Main Responsibility

  • Focus on people’s strengths and community assets, by listening and connecting people, their families, and carers.
  • Develop highly personalised conversations and innovative approaches to needs and care planning.
  • Manage own workload and take responsibility for producing work to agreed deadlines.
  • Partnership working with colleagues in adult social care and with other key professionals/agencies.
  • Ensuring all work and interactions are recorded accurately and in a timely manner.

The Ideal Candidate

  • Someone who has good communication and listening skills.
  • Someone who is practical and organised.
  • Someone with computer skills and is confident with using IT systems.
  • Able to work independently, using your own initiative.
  • Access to a vehicle to use for work.
  • If you have past experience in this area of work great, if not, don’t be put off as full training and support will be provided – we need compassionate, dedicated people who want to help others.
