FULL DESCRIPTION

[Employer hidden — view at passion-project.co.uk] is seeking an enthusiastic, motivated, and adaptable candidate to join our team. The role of Care Coordinator is dynamic and demanding. We are looking for someone who is confident, highly organised, and able to manage multiple tasks while communicating effectively in a fast paced healthcare environment.

Main duties of the job

  • Organise, attend, and minute Multi-Disciplinary Team (MDT), Complex Care, and Safeguarding meetings; follow up on actions.
  • Prepare personalised care plans (PCPs) in collaboration with GPs and managers.
  • Record patient interventions accurately on electronic databases (e.g., EMIS).
  • Act as a key point of contact for Safeguarding and follow up on related actions.
  • Provide support and guidance to patients, families, and carers, ensuring a patient-focused approach.
  • Work collaboratively with internal teams and external services to maintain efficient care delivery.
  • Plan and prioritise own workload, using initiative while contributing effectively to the team.
  • Maintain strong IT skills, including Microsoft Office, Outlook, and Excel, to manage administrative tasks.

– Responsible for preparing, arranging, attending and minuting Multi-Disciplinary Team Meetings, Complex Care and Safeguarding meetings. (scheduled monthly for the year) – Proactively prepare any actions prior to the MDT ensuring all relevant clinicians are present. – To record patient interventions on relevant electronic database systems (e.g. EMIS). – Follow up on all forward actions resulting from MDT discussions. – Create Personalised Care Plans for patients, working with GPs and managers to identify patients that would benefit from a PCP. – To be customer (patient, carer, GP) focused when representing the service. – To work collaboratively with other teams and services to maintain an effective and efficient service. – Point of contact for Safeguarding. – To offer appropriate support and guidance to patients and their families/carers. – To plan / organise work using own initiative, whilst being able to work as a valuable member of a team. – To have excellent IT skills, to include Microsoft Office, Outlook and Excel. – To action emails that are sent through the database for Adult and Child Safeguarding. – Padstow/St Columb Safeguarding Meeting every 6-8 weeks. – Complex Care Meeting every 4-5 weeks. – Excellent customer service skills and the ability to put the needs of patients first. – Computer literacy, including a working knowledge of relevant programs, such as Word and Excel. – The ability to function well in a team to provide effective administrative support. – Excellent organisational skills to ensure that an office runs smoothly. – Ability to maintain strict confidentiality and deal with the public in a sensitive way. – Good spelling and grammar for typing clinical and administrative letters and other correspondence. – Knowledge of medical terminology to accurately type out medical reports and other documents
