Case Manager

Based in Leicestershire, [Employer hidden — view at passion-project.co.uk] provide bespoke case management and expert witness services for those who have sustained a spinal cord injury, amputation or other severe orthopaedic injuries. We have an exciting opportunity for an experienced and highly organised case manager to join our small but dedicated team.  The role is based from home managing clients within a 90 minute radius.

As a case manager you will address current and future needs of clients with spinal cord injury, amputation or other complex orthopaedic injuries, collaborating with multidisciplinary team (MDT) members, including the legal team, clinicians and other service providers, to promote maximum independence.

Main responsibilities include:

  • Preparing an Immediate Needs Report (INR) following detailed assessment of the client’s needs, goals and expected outcomes.
  • Identifying care, transport, therapy, equipment and housing needs and supporting the client to address these issues.
  • Arranging and attending meetings, appointments and assessments as required.
  • Recommending therapeutic and rehabilitation interventions and projecting costs.
  • Writing costed case management reports and being responsible for all communication regarding the client and the sharing of information regarding their progress and ongoing needs to insurers, solicitors, family members, physicians, other treating team members and the EWA office.
  • Leading on the recruitment and management of support workers, including development of client focused care plans, care provision, supervision and team meetings.
  • Maintaining accurate and up to date client records such as on Qunote and in support worker folders.
  • Complying with EWA policies and procedures, relevant legislation, associated regulatory bodies and guidelines and keeping up to date with all changes. Be individually accountable for standards of professional practice.

SKILLS AND COMPETENCIES

  • A minimum of 10 years professional experience as an occupational therapist, case manager or nurse which includes work with spinal cord injured individuals and work within the private industry.
  • Excellent report writing skills with a high level of accuracy.
  • Excellent interpersonal skills. Be honest and caring and have the ability to develop relationships with the client, their legal team, clinical professionals, service providers and care teams.
  • Be organised and have the ability to multitask, make decisions and work with a consistent and coordinated approach.
  • Excellent communication skills, positive attitude and a team player.
  • Ensure that the client’s dignity, privacy and overall welfare is paramount and respected at all times.
