Due to our continued growth, we are pleased to be recruiting an additional Field Care Supervisor to join our existing team in Welwyn Garden City. We are looking someone who enjoys working with a team of Care Assistants to support the care of our service users. We deliver outcome focused care and support and focus on a high quality service allowing plenty of time to meet service Users needs. You will be carrying out Spot checks, completing care assessments, onboarding and mentoring our new care workers as well as delivering care when required.

The role:

We are looking for a passionate individual with a wealth of care experience and knowledge to support our Care Assistants and Service Users across our Hertfordshire branch. Within this role you will be proactive and responsive to ensure that our service users receive our outstanding care deliver standards. This role will require you to work closely with our operational team and our field based care assistants to communicate and deliver on our company values.

  • Complete assessments with our service users and develop personalised person-centred care plans
  • Complete checks to identify any changes of requirements and amend care plans appropriately.
  • Audit medication charts regularly highlighting any additional training requirements.
  • Complete coaching and mentoring of care assistants in the company.
  • Attend new referrals and advise service users on different care options available to them.
  • Build and maintain professional relationships with all areas of [Employer hidden] and external stakeholders

Person Specification:

  • Minimum of 2 years care experience.
  • NVQ level 2 or above in Health and Social Care or prepared to work towards
  • Ability to complete and maintain records and reports e.g. service user assessments and reviews.
  • Holding a full driving license and access to your own vehicle.
  • The ideal candidate for this role will approachable, caring, kind.
  • Be self-motivated
  • Strong attention to detail and be a completer/finisher.
  • A team player with strong interpersonal skills.
  • Strong Communication skills.
  • Ability to both lead and be part of a team providing direct care and support.
