Job Description

Position Summary:

The First Contact Practitioner provides specialist MSK physiotherapy assessment and treatment in a mixture of clinic-based settings, working as an autonomous practitioner and working in GP surgeries/primary care networks as a member of a multi-disciplinary team.

You will also assist with service development and training programmes. Our mixture of services provides:

  • Face-to-face appointments
  • Virtual appointments (video and telephone)
  • Digital MSK support for front line conditions

Job Purpose:

  1. Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions). This will involve seeing patients, without prior contact with their GP, to establish a rapid and accurate diagnosis and management plan.
  2. Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of diagnostic test services such as x-rays and blood test and interpret and act on results to aid diagnosis and the management plans of patients.
  3. Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.

Main Overview of Role:

- ‍To independently provide physiotherapy assessment to formulate a clinical diagnosis and an appropriate treatment plan which places the patient at the centre of care using evidence-based and best practice approach.

- Utilise support from a senior physiotherapist where required, recognising own limitations.

- Maintain physiotherapy records in line with HCPC regulations.

- Timely completion of medical reports when required.

- Participate in training to ensure CPD maintained in line with professional standards.

- Excellent communication with staff, patients and relatives.

- To be involved in the development of new FCP clinics and service.

- Co-ordinate Annual Leave with other PCN FCP staff and PCN lead FCP.

- Work alongside the support from PCN lead FCP's and the management team.
