 As a CAS Clinician, you will play a pivotal role in delivering expert, person-centred care to frail individuals, helping to reduce hospital admissions and improve health and social care outcomes.

Role Overview

  • Conduct comprehensive assessments of frail patients in the community to identify needs and risks
  • Develop and implement personalised care plans focusing on frailty prevention and management
  • Provide rapid crisis response to support patients in their own homes and avoid unnecessary hospital admissions
  • Coordinate discharge planning and support to ensure safe and effective transitions from hospital to community care
  • Work collaboratively with a multi-disciplinary team including frailty nurses, therapists, social prescribers, GPs, dementia specialists, and more
  • Offer guidance and support to patients, families, and carers to enhance independence and quality of life

Requirements:

  • Experience in community or frailty care (e.g., nurse, therapist, or allied health professional)
  • Skilled in comprehensive assessment, care planning, and crisis management
  • Excellent communicator, able to build rapport with patients, families, and colleagues
  • Passionate about delivering high-quality, holistic care tailored to individual needs
  • Comfortable working in a fast-paced, collaborative environment
