Streaming Practitioner

As a Streaming Practitioner, you will assess patients presenting to urgent care, using your clinical judgement to determine the most appropriate pathway. You will safely diagnose, refer, discharge, or signpost patients in line with their clinical needs.

[Employer hidden] has opportunities for Streaming Practitioners to deliver high-quality clinical care within an Urgent Treatment Centre (UTC) in Sunderland.

As a Streaming Practitioner, you will assess patients presenting to urgent care, using your clinical judgement to determine the most appropriate pathway. You will safely diagnose, refer, discharge, or signpost patients in line with their clinical needs.

Key responsibilities

  • Undertake assessment of patients within scope of competence
  • Take brief clinical histories and record observations
  • Make safe, timely decisions regarding patient pathways in urgent and emergency care
  • Signpost patients to appropriate services based on clinical need
  • Work autonomously while contributing effectively to the wider clinical team

Shifts

  • Weekdays, evenings, weekends, and bank holidays
  • Shift times: 10:00–20:00

Qualifications & Experience

  • Registered Nurse or Registered Paramedic
  • Minimum 2 years post-registration experience, including at least 1 year in:
  • Primary Care, NHS 111, ED, UTC, MAU/EAU, or other first-contact setting
  • Strong clinical assessment and decision-making skills
  • Advanced assessment skills (or willingness to undertake training)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Confident using IT systems and clinical documentation tools
