Stroke Community Support Worker

You will play an important part of shaping the service and being an integral of the team from the start! If you have an interest in supporting patients returning home from hospital, who have suffered with a Stroke and need to rehabilitate within their own environment, then this unique job role and will definitely provide you with job satisfaction.

You will play an important part of shaping the service and being an integral of the team from the start! If you have an interest in supporting patients returning home from hospital, who have suffered with a Stroke and need to rehabilitate within their own environment, then this unique job role and will definitely provide you with job satisfaction.

This 6-week service supports patients who have had a Stroke, you will have a pivotal part to play, which aims to enhance their potential for recovery. You will work closely with a multi-disciplinary team, such as Physiotherapists, Occupational Therapists, Speech and Language Therapists, Specialist Nurses. The successful candidate will be involved in supporting and promoting their personal activities of daily living, which may include regular use of selected exercises will aid the patients in building up their muscles, improving abilities, well-being and to improve their quality of life.

We are ideally looking for individuals who have previous care experience with an interest in neurological conditions and happy to work autonomously and as part of a team. Ideally the candidate will hold a Health and Social Care Qualification and already completed their Care Certificate. Due to the nature of working within the community time management is essential.
