Position Summary: The Community Complex Care Worker provides high-quality, individualised care to clients in the community with complex physical, medical, and social needs. The role includes hands-on support, advocacy, and collaboration with healthcare professionals, families, and community services to ensure holistic care.
Key Responsibilities:
- Deliver direct, person-centered care to clients with complex needs, including personal care, medication support, and health monitoring.
- Support clients with tracheostomies, Non-Invasive Ventilation (NIV), and Cough Assist devices as required.
- Develop and implement individualized care plans in consultation with clients, families, and the Registered Complex Nurse.
- Advocate for clients to ensure access to appropriate healthcare, social services, and community resources.
- Monitor and document clients’ health status, progress, and any changes in condition.
- Facilitate social engagement and community participation to promote overall well-being.
- Respond promptly to emergencies or urgent care needs.
- Maintain confidentiality and comply with all relevant policies, procedures, and regulations.
- Participate in team meetings, training, and professional development activities.
Qualifications & Experience:
- NVQ Level 2 in Health and Social Care desirable but not essential.
- Experience working with clients with complex needs is desirable.
- Knowledge or experience in supporting clients with tracheostomies, NIV, and Cough Assist devices is highly advantageous.
- Strong understanding of person-centered care and community support.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Current first aid and CPR certification (or willingness to obtain).
- Valid UK driving license and reliable transport preferred.
