About the Role:

The role of Care Coordinator is pivotal in the day to day running of our busy, established Heywood office. Working closely with the management team, you will be responsible for the planning, coordination, and ongoing management of care packages, ensuring seamless service delivery to our valued clients.

You will be based at your local [Employer hidden] office in Heywood, but travel across the Northwest region will be an essential part of this role. Therefore, we are seeking applicants who hold a Full UK Driving Licence and have access to their own vehicle. Candidates must be comfortable driving long distances and in different conditions, including motorway use.

Key responsibilities include:

  • Act as the first point of contact for our clients, their families, and healthcare professionals.
  • Effectively manage rotas for your care teams, responding efficiently to any day-to-day changes
  • Monitor care delivery to ensure it meets individual care plans and quality standards.
  • Actively participate in multi-agency meetings with Integrated Care Boards (ICBs), advocating for client needs and contributing to care planning decisions.
  • Support the out-of-hours, on-call service – with no requirement for hands-on care.

What You will Need

  • A Full UK Driving Licence and access to your own vehicle, plus confidence to travel across the Northwest region (This will include motorway driving)
  • Proven experience within a care coordinator role.
  • Proven experience of scheduling and rota management, as well as being familiar with using relevant home care software, such as people planner.
  • Solid understanding of person-centred care and planning.
  • Strong organisational skills, with the ability to manage multiple care packages and respond quickly to changing priorities.
  • Confident in making decisions with the ability to think on your feet and solve problems while staying calm and professional.
  • Clear communication skills, with the ability to adapt your style to suit different audiences, including families, professionals, and external partners.
  • Comfortable negotiating and influencing, particularly when managing rotas or balancing client needs.
  • A team player with a positive, proactive approach and the ability to build strong working relationships.
