Job Overview: The Care Coordinator will be responsible for the smooth and efficient scheduling, planning, and management of care services for clients, ensuring high-quality care is delivered by support staff. This role is suited for individuals with excellent organizational and communication skills who enjoy working in a dynamic, remote environment.

Key Responsibilities:

  • Coordinate care schedules and allocate care workers to clients.
  • Maintain and update client records, ensuring compliance with legal and company standards.
  • Communicate with clients, families, and care teams to ensure needs are met.
  • Liaise with healthcare professionals, social services, and other agencies to ensure holistic care is provided.
  • Provide administrative support, including managing appointments and ensuring staff availability.
  • Monitor the performance of care staff and address any issues or concerns.
  • Ensure all care documentation is accurate and up to date, following confidentiality guidelines.
  • Provide support and guidance to care staff and assist with training or development as needed.

Skills & Qualifications:

  • Experience in a care coordination or similar administrative role.
  • Strong organizational and time-management skills.
  • Excellent communication skills, both verbal and written.
  • Knowledge of care management software or CRM systems is an advantage.
  • Ability to work independently in a remote or hybrid setting.
  • Understanding of health and safety regulations and care standards.
