Requirements:

• Proven experience in a care coordination or similar role.
• Strong understanding of care planning, case management, and healthcare regulations.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team and independently.
• Strong organizational and problem-solving skills.
• Compassion, patience, and a commitment to providing high-quality care.
• Proficiency in using care management software and other relevant technology.
• CPR and First Aid certification is a plus.
