Postion Overview

You will coordinate care plans, liaise with healthcare professionals, and ensure our clients receive the services they need. Your role is vital in maintaining the well-being and satisfaction of both our clients and our care staff.
Key Responsibilities:
- Develop and manage individualized care plans for clients, ensuring they receive appropriate services.
- Coordinate with healthcare professionals, social workers, and families to provide comprehensive care.
- Monitor and assess the ongoing needs of clients, adjusting care plans as necessary.
- Ensure compliance with all relevant regulations and best practices in care delivery.
- Maintain accurate and up-to-date records of client care and progress.
- Support care staff by providing guidance, training, and supervision.
- Address any issues or concerns raised by clients or their families promptly and effectively.
- Participate in regular team meetings and contribute to the continuous improvement of care services.
- Manage schedules and allocate resources to ensure efficient and effective care delivery.
Requirements:
- Bachelor’s degree in Social Work, Nursing, Healthcare Management, or a related field.
- Proven experience in a care coordination or similar role.
- Strong understanding of care planning, case management, and healthcare regulations.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team and independently.
- Strong organizational and problem-solving skills.
- Compassion, patience, and a commitment to providing high-quality care.
- Proficiency in using care management software and other relevant technology.
- CPR and First Aid certification is a plus.
