Do you want to make a real difference to people for serious mental illness? This exciting role will make a real difference, promoting the uptake of annual health checks for patients registered with Serious Mental Illness (SMI) across all our 19 GP Practices in Calderdale. Building on existing efforts this role will also support practices in Calderdale to in deliver the highest quality SMI Health Checks for their patients.

Working closely with our GP Practices, Mental Health Practitioner roles, Personalised Care Teams, VCSE and patients registered with an SMI promoting personalised care and addressing health inequalities.
