As an agency Support Worker/Care Assistant, you will be responsible for
• Assessing, planning, implementing, and evaluating patient care plans in collaboration with the healthcare team. • Administering medications and treatments according to established protocols and physician orders. • Monitoring and documenting patient vital signs, symptoms, and medical histories. • Performing diagnostic tests and analyzing results to provide accurate patient assessments. • Assisting with patient admissions, discharges, and transfers. • Educating patients and their families about medical conditions, treatments, and preventive healthcare practices. • Collaborating with interdisciplinary teams to ensure effective and coordinated patient care. • Maintaining accurate and up-to-date patient records and documentation. • Adhering to legal, ethical, and professional nursing standards.
To be successful in this role, you will need:
• A minimum of 6 months of experience as a care assistant, preferably in a care home setting • A passion for providing person-centred care and enhancing the quality of life of residents • A flexible and adaptable approach to work, with the ability to work shifts, including nights and weekends, as per the agency’s rota • A positive attitude and a commitment to continuous improvement • A valid DBS check or willingness to obtain one
