Summary
A Clinical Nurse Assessor is required by [Employer hidden — view at passion-project.co.uk] Solutions. This is an excellent opportunity for a Qualified band 5 Nurse, to utilise their clinical knowledge and clinical skills to further their career. You will join an established home care team to lead and undertake assessments on a daily basis to determine the individual needs of potential and current clients. The main objective is to ensure a quality-of-life-driven care plan, promoting independence and autonomy where possible.
Key Responsibilities/Duties
- Lead and undertake assessments on a daily basis.
- Utilise clinical and evidence-based knowledge, interpersonal techniques and skills to determine the individual needs of potential and current clients.
- Carry out comprehensive assessment reviews and analysis.
- Complete a robust, person-centred care plan and risk assessments.
- Promote best practices and provide professional advice to the home care team.
Core Requirements/Qualifications/Skills
- Qualified Nurse
- 1+ years post-qualification experience
- Valid registration on NMC
- Excellent report writing skills with attention to detail.

Are you compassionate about patient care and quality care and looking to make a real impact? We’re thrilled to announce a fantastic opportunity to join our team as a Clinical Nurse Assessor!
This is an excellent opportunity for a Qualified band 5 Nurse, to utilise their clinical knowledge and clinical skills to further their career.
Responsibilities
You will join an established home care team to lead and undertake assessments on a daily basis. The role allows you to utilise your clinical and evidence-based knowledge, interpersonal techniques and skills to determine the individual needs of potential and current clients.
Carrying out comprehensive assessment reviews and analysis will enable you to complete a robust, person-centred care plan and risk assessments, containing a detailed history of clients’ condition(s) including information gathered on the functional impact of their health condition or impairment on their daily living and mobility, their current medication and treatment.
The main objective is to ensure a quality-of-life-driven care plan, promoting independence and autonomy where possible
You will promote best practices and provide professional advice to the home care team, promoting good relationships and liaising with colleagues from other disciplines both internally and externally.
Requirements
- Qualified Nurse,
- 1+ years post-qualification experience
- Valid registration on NMC
- Excellent report writing skills with attention to detail.

- Nursing: 1 year (required)
